If you are transferring from another practice in New Zealand, we can request your medical records on your behalf.

If you prefer to fill out our paper enrolment forms, please ask our reception staff.

We will need a copy of your New Zealand Passport or New Zealand Birth Certificate (with a form of photo ID). If you don’t have appropriate ID, please visit reception to discuss your options

When you enrol with us your fees are cheaper because we receive finance from the government to subsidise the cost of your visit.
